NEWS RELEASE
Pocono Medical Center to provide H1N1 vaccine to individuals in high-risk groups
 

What: Pocono Medical Center staff will administer the H1N1 vaccine to individuals who fall into the following groups:

  • Pregnant women
  • Household contacts and caregivers for children younger than 6 months of age
  • Healthcare and emergency medical services personnel
  • All people from 6 months through 24 years of age
  • Persons aged 25 through 64 years who have health conditions associated with higher risk of medical complications from influenza

Where: Mountain Healthcare Center, 100 Community Drive, Tobyhanna, PA 18466
                       
When: December 19, 2009 and December 20, 2009 from 8:00 am – 12:00 pm  

Additional Information: Individuals who fall into the high-risk categories that are interested in receiving the H1N1 vaccine must make an appointment by calling (570) 422-8281 during the hours of 8:30 am and 4:00 pm on Monday, December 14 – Thursday, December 17, 2009.  Individuals can also call on Friday, December 18, 2009 from 8:30 am to 12:00 pm.  We would like to remind you that you must make an appointment by speaking with one of our staff members and urge you to not leave a message on the voicemail.  Please know that we are expecting a high number of calls and would appreciate your patience and understanding.  All individuals will be required to complete an H1N1 Consent Form (child and adult) and a Vaccine Record Form for the Pennsylvania Department of Health.
