APPOINTMENTS, FEES & REFERRALS
Appointments: Appointments are available on weekdays by appointment only. Saturday appointments are also available (once per month). Please arrive about 20 minutes early for your appointment to complete necessary registration paperwork.

Fees & Referrals: Payment is required at the time of service. Patients with insurance are asked to pay co-payments, co-insurance, deductibles and charges for non-covered services at the time of their visit. Many managed care plans require written referral or pre-authorization for surgery or other procedures. Our staff can help you with your plan requirements.
