Bringing Quality Care to the Comfort of Your Home
Home care is an essential part of health care today, touching the lives of nearly every American. Staying in the home provides a restful, healing environment in which individuals can remain independent while receiving the care they require. Recovery, rehabilitation and chronic diseases can be cared for in the home. And, when a new or recurring problem occurs, it can be handled promptly and reported to your physician.
When you need home health care services for yourself or a family member, choose Pocono Health System Visiting Nurse and Hospice. Highly trained individuals provide home care services under the direction of the patient's physician and offer the peace of mind that is so important when your health or that of a loved one is a primary concern.
Pocono Health System Visiting Nurse and Hospice is a member of a 110-year-old network of community-based, not-for-profit home health agencies that provide care for patients and families with commitment and efficiency. We have the resources, skill and compassion on which you can depend, 24 hours a day, seven days a week.

Skilled Nursing Care
At the heart of our services is skilled nursing. Our experienced staff is trained to meet the special challenges presented in the home environment. Our nurses work directly with physicians and other members of the health care team to develop the most effective treatment plan for the patient. We offer the compassionate, innovative, quality care that is essential to a patient's health and well being.

Rehabilitative Therapy
Our staff of licensed physical, occupational and speech therapists are available to provide rehabilitation therapy as prescribed by the physician. Patients progress more rapidly when therapy is provided in the comfort of their homes.

Patient Education
Sometimes the most difficult part of coping with an illness is trying to understand it. Our professionals are experts at educating patients and their families about disease processes, treatments, therapies and/or medication plans. Using comprehensive educational material, patients learn about their disease and treatment, and how their involvement can help improve their condition and enhance their quality of life.

Infusion Therapy
Our staff can provide infusion or teach the patient and their family how to safely administer the treatments in the home setting.

Counseling Support
In cases where cognitive or emotional problems result from an illness, injury, or change in life situation, we can provide professional counseling including pastoral services. We work closely with the physician to ensure that the most appropriate support is provided.

Home Health Aide Services
Our trained and certified home health aides can take care of the little things that make a big difference in a person's quality of life. They assist patients with activities of daily living such as bathing, dressing and ambulating. The comfort and peace of mind that these important tasks provide are invaluable.

Meal Preparation
Regular, nutritious meals are an important part of improving or maintaining one's health. Our home health aides are trained to prepare simple meals to ensure patients are following dietary guidelines determined by their physician.

Assessments & Evaluations
Our highly-skilled professionals are trained to assess the home care needs of individual patients to determine what services are required. You or your family member may call us directly for a free in-home evaluation.
