Diabetes

Self-management education is an essential part of diabetes treatment. Combined with medical and nutritional therapy, education gives the learner the ability to become an active participant in his/her care. Well-managed diabetes has been shown to prevent and/or delay the acute and chronic complications of diabetes and reduce the number and length of hospital admissions.

Pocono Medical Center’s Success With Diabetes* Self Management Program is conducted by certified diabetes educators, registered nurses and dietitians. Participants learn the skills needed to live a healthy lifestyle. A physicians order is required to attend. Most insurance companies cover all or part of diabetes education and equipment costs.  As plans vary, contact your insurance carrier to determine your coverage by calling the customer service number on your card.

For more information or to register for the program, please call the Diabetes Education Department, please call (570) 476-3395.
