Patient Registration


The Admissions & Outpatient Registration office is accessed through the main entrance of PMC's main building on East Brown Street and is located on the first floor of Area B. Our new area provides private interview offices for your comfort and privacy.

So as to not delay the registration process, please bring:

    • Picture/Military ID
    • Insurance card(s)
    • Know your current personal information (address, phone number, etc)
    • Co-pay or deductible
    • Insurance referral and/or pre-certification approval number if required
    • Know your primary care physician information (name, address, phone number, etc)
    • Physician orders and/or prescription.

A complete physician's order must have the following six elements:

    • Patient's name
    • Date
    • Tests to be performed
    • Diagnosis code or signs and symptoms
    • Physician's name
    • Physician's signature

Outpatient laboratory hours are 6am to 6pm Monday through Friday and 6:30am to 2pm on Saturday. If you need lab tests drawn after hours, you may experience delays while waiting for an available inpatient phlebotomist. Patients requiring registration after 9pm should go to the Emergency Room.

To pre-register for scheduled Radiology procedures call 570-476-3312

To pre-register for scheduled Radiology procedures such as CT Scan, MRI, Ultrasounds, Mammograms, please call at least 24 hours prior to your scheduled procedure. 

Please have the following ready when you call us:

  • Your physician's order
  • Insurance cards
  • Referral (if required) 

Your insurance card is always required to ensure accurate billing and good customer relations. We will bill most insurance companies, however, the individual must check with their private health insurance to make sure the hospital is in their company's network to avoid large deductibles or hospital bills. If you are a member of an HMO, they may require you to obtain treatment from certain providers. They may also require a referral or pre-certification. Pre-certification is often required before registration can be completed. Look for a telephone number on your insurance card to contact your insurer about its requirements for pre-certification.
